1. Form Content Preparation Guidelines
To ensure we can create accurate and user friendly forms for your website, please use the following guidelines when preparing your content. This document outlines the key elements we require for each form. Please provide these details for each individual form you wish to include on the site.
1.1 Form Title
- What to provide: A clear, concise title for the form
- Example: Contact Us, Event Registration, Feedback Form
1.2 Introductory Text
- What to provide: A brief paragraph that explains the purpose of the form or gives users instructions on how to complete it
- Example: Please complete the form below to register your interest. All fields marked with * are required.
1.3 Questions / Form Fields
For each question or field, please provide the following details:
1.3.1 Label (Question Text)
- The text that will be shown to users
- Example: Full Name, Email Address, How can we help you
1.3.2 Field Type
Choose from one of the following standard types:
- Single Line Text
- Paragraph Text
- Drop down (select one)
- Checkboxes (select multiple)
- Radio Buttons (select one)
- Date Picker
- Time Picker
- File Upload
- Number
- Hidden Field (if needed for internal tracking)
- Others (please specify)
1.3.3 Placeholder Text (Optional)
- Light grey example text inside the field to guide users
- Example: For Email, placeholder might be you@example.com
1.3.4 Instructions or Help Text
- Short instructional note shown beneath or beside the field
- Example: Please use your work email address
1.3.5 Tips (Optional)
- Tooltips or expandable help boxes the user can click on or hover over for additional guidance
- Example: We will not share your information with third parties
1.3.6 Required Field
- Indicate whether the field must be filled in to submit the form
- Yes or No
1.3.7 Validation Rules (if applicable)
- Specify any format or length requirements
- Example: Must be a valid email address
- Minimum 10 characters
1.3.8 Validation Error Message
- Message to display if the user input does not meet the validation rule
- Example: Please enter a valid email address
1.4 Conditional Logic (Optional)
- Do any fields appear based on earlier answers
- Example: If the user selects Yes to Do you require accommodation, then show fields for accommodation details
1.5 Confirmation Message
- What message should be shown to users after successful form submission
- Example: Thank you for contacting us. We will get back to you within 2 business days
1.6 Notification Emails
- Should the form send an email when submitted
- Who should receive it (name and email)
- What should the subject line be
- Should the form submitter receive a copy (confirmation email)
- If yes, provide wording for the confirmation message or email
1.7 Redirect After Submission (Optional)
- Should the user be redirected to a thank you page or another URL after submitting
- If yes, provide the destination URL
1.8 File Upload Guidelines (if applicable)
- What types of files are allowed
- What is the maximum file size
- How many files can be uploaded
1.9 GDPR or Consent Checkbox (if required)
- Provide the exact wording for any consent statements required
- Example: I consent to my data being stored and used for the purposes outlined above
- Specify if ticking this box should be mandatory before form submission
1.10 Other Notes or Special Requirements
- Any additional elements or instructions you would like us to be aware of
1.11 Submission Format
Please supply the above information for each form in one of the following formats:
- Word Document (.docx)
- Google Doc (with sharing permissions enabled)
- Spreadsheet (if preferred for structured data)
- PDF (only if other formats are not possible)
